While attending the Philly Friendship Walk, I acknowledge that on behalf of myself and/or my minor child(ren), I hereby waive, release and discharge any and all claims including but not limited to injury, illness and damages caused by or related to participation in The Philly Friendship Walk and related events. I understand that in doing so, all risks relative to such claims including but not limited to injury, illness or damages are hereby assumed by me, whether negligently or intentionally inflicted, and that I covenant not to sue or make claims for the same against the sponsors, including but not limited to The Friendship Circle-Philadelphia Region Inc., Lubavitch House/Guideline Services, Radnor School District, Radnor Township, and Brandywine Realty Trust, event crew, active members, organizers, employees, agents or representatives of the organization or event. I agree this waiver is given in exchange for permission to participate in the Philly Friendship Walk and related events. I also grant permission for the use and reproduction of any photographs and any audio and video recordings depicting me and/or my minor child(ren) for use in legitimate accounts and promotions of the Friendship Circle Philadelphia region Inc.

I hereby consent to and permit emergency treatment in the event of injury or illness while participating in the Philly Friendship Walk. 

The Friendship Circle Philadelphia region Inc. reserves the right to refuse or dismiss anyone that may cause any disturbance or hindrance in any manner that could jeopardize the safety of oneself or others.
