You consent for your entire participating family to the following release of liability statement on behalf of the Fountain International Magnet School, The City of Pueblo, and all those involved in race organization. You also consent that you have reviewed your registration information and determined that the age, gender, and name are correct. “I hereby release the City of Pueblo, Fountain International Magnet School (FIMS), and all municipal agencies whose property and/or personnel are used, and other sponsoring individuals and/or companies from responsibility for any injuries or damages I may suffer as a result of my participation in the Lions Roar for Fitness 5K and 2.5K Challenge and related events. I hereby certify that I am in good physical and mental condition and am able to safely complete this event. I hereby consent to receive emergency medical treatment at my own expense, which may be deemed necessary in the event of injury, accident or illness during this event. I will additionally permit the use of my name and pictures in broadcasts, telecasts, newspapers, brochures, etc. I understand that my entry fee is non-refundable and I agree to abide by the rules of this race. If I do not follow the rules of the event, I understand that I may be removed from the event. As a participating athlete, I certify that all information provided in the form to be accurate and complete.
